Public Health
Clinical Advisor, Diabetes & Hypertension
About this role
D-tree International is recruiting a Clinical Advisor Consultant to guide the design of a collaborative care model for diabetes and hypertension in Tanzania Mainland. The assignment supports D-tree's effort to move screening, follow-up, and adherence support closer to communities through the National Integrated CHW Programme. The consultant will review and validate clinical protocols, including the Standard Treatment Guidelines and National Essential Medicines List, Ministry of Health NCD guidance, the CHW curriculum, and the clinical logic in D-tree's digital tools, testing them against WHO PEN and HEARTS recommendations. The advisor will define who community health workers should screen, which thresholds should trigger referral from community to facility and from primary facilities to district hospitals, and which post-diagnosis tasks can be shared safely.
Further work involves mapping Tanzanian evidence on care gaps and failure points across the cascade, then translating findings into revised workflows, decision-support logic, referral rules, and counselling messages during co-design workshops with D-tree, the Ministry of Health, and PMO-RALG clinicians. Applicants must hold an MD or equivalent with a postgraduate degree in internal medicine, family medicine, or public health, and be registered with the Medical Council of Tanzania. At least seven years managing diabetes and hypertension in Tanzanian public health facilities is required, alongside strong knowledge of national treatment guidelines, NEMLIT, primary care structures, and qualitative or mixed-methods research including key informant interviews. Preferred experience includes community-based NCD programmes, WHO PEN or HEARTS implementation, digital health or clinical decision-support tools, and health outcome measurement.
This is a short-term consultancy of four to eight weeks beginning 1 November 2026, with about 15 days of effort including five days of fieldwork. The advisor reports to D-tree's Senior Technical Advisor for Tanzania Mainland and works with design, digital, and health economics colleagues. A proposed budget in TZS must accompany the application, and applications close on 28 October 2026.
